Welcome to Great Waves Swimming Academy! Before your first lesson we need to collect some information for the swimmer(s), including parent/guardian contact details, medical information and any additional needs. This helps us to:

Become familiar with the swimmer
Provide key information to your instructor
Plan lessons to best meet your swimmer's needs

Does the swimmer have any additional needs, disabilities or support needs that may affect their swimming lessons? This can include diagnosed or undiagnosed needs.

Safety and Water Safety Awareness

This helps us understand the level of supervision and safety awareness the swimmer needs around water.

Is the swimmer likely to leave the pool, poolside or teaching area unexpectedly?

Is the swimmer likely to enter the water without being instructed to do so?

Does the swimmer understand and respond to safety instructions such as "stop", "wait" and "come back"?

Is there anything the swimmer may do when distressed that could present a risk to themselves, other swimmers or the teacher?

Should the teacher be aware of any challenging behaviour?

Behaviour, Anxiety and Emotions

We want to understand how to support the swimmer in a new environment and how to manage any challenging behaviour.

Does the swimmer experience anxiety? Are there strategies to help?

What behaviour indicates the swimmer is upset or distressed, and are there strategies to help? e.g. crying, biting hands, running away

Are there routines or transitional objects the team should be aware of? e.g. a pre-pool routine or comfort toy

Can the swimmer be aggressive or physically lash out?

What are the early signs that the swimmer is becoming anxious, overwhelmed or dysregulated?

Are there any known triggers?

What usually helps the swimmer to regulate or calm?

What should the teacher do if the swimmer becomes significantly distressed?

What should the teacher avoid doing when the swimmer is distressed?

Does the swimmer ever experience meltdowns or shutdowns?

Can the swimmer become physically aggressive towards themselves or others when distressed?

Speech, Language and Communication

Please provide information if the swimmer has any difficulties with speech, language or communication.
